PREPARING FOR BIRTH AND BABY

You should have these supplies and the answers to these questions by the 33rd week of pregnancy

GBS

Please read the section on GBS

Vitamin K

Please read the section on Vitamin K deficiency

  • ☐ I do not want my baby to have the vitamin K injection 
  • ☐ I want my baby to have the vitamin K injection 
    • ☐If so, and are birthing at home, purchase the Vitamin K add-on when you order your birth kit. 

Newborn Screening

Please read the “The Newborn Metabolic Screen” in your prenatal guide. 

☐ I do not want my baby to have the metabolic screening test

☐ I want my baby to have the newborn screening test. 

☐I want to do the test through my pediatrician

☐ I want you to do it through the state lab ($150)

☐Purchase the newborn screen add-on with your kit and place a check made out to Midwives Rising in the amount of $150 and place with your birth supplies, if birthing at home. If birthing at the center, discuss payment with your midwife. 

☐ I want you to do it through the private lab ($89)

☐Purchase the newborn screen add-on with your kit and place a check made out to Perkin-Elmer in the amount of $89 and place with your birth supplies, if birthing at home. If birthing at the center, discuss payment with your midwife.  

Ultrasound. Late ultrasound can be useful in determining that baby is in a good position for birth.

☐ I do not want a late ultrasound and I am comfortable delivering a surprise breech at home or birth center.

If you are not married, do you want the biologic father to be on the birth certificate?
