BIRTH CERTIFICATE AND SOCIAL SECURITY CARD

With your signature and permission, the Social Security Card will be automatically mailed to you.

You must purchase a copy of the Birth Certificate; Vital Records will not allow us to do that for you. 

Once the birth is complete:

- We will fill out the Certificate of Live Birth Worksheet 

- The mother or father will sign the Worksheet, indicating that the information is correct. 

- The primary midwife will sign the Worksheet, indicating that she witnessed the birth. 

- If mom and dad are not married and they want dad's name to be on the birth certificate, they will need to fill out an Acknowledgment of Paternity form. You will need 3 copies of this form.

- Within one week of the birth, our office will send the Worksheet to Vital Records

- Vital Records will then register the birth with state & federal agencies, including the Social Security Department if the parents indicated that they wanted the Social Security Card to be automatically issued.

- When the Social Security Department receives the data from Vital Records, they’ll mail the baby's card to the parents' address.

- Receiving the card is a good indication that your copy of the Birth Certificate is ready for purchase. 

- You have three options for purchasing a copy of the birth certificate.

1) Online:

2) By mail:

Send an application, $20 money order or cashiers check, a self-addressed stamped envelope, a copy of the front and the back of your drivers license, proof of relationship (prenatal labs) and the application (provided on the next page) to your county’s vital records office (below). You should receive your copy within 7 business days. 

3) In person w/ same day service. Call ahead to verify acceptable forms of payment. Take your payment and your I.D. with you to your local County Health Department Office of Vital Records. You may want to call ahead and see if they need anything else from you at the time of order.
